This form is used to request modifications to an existing Purchase Order or to close all or part of a Purchase Order. Changes may include updates to quantities, pricing, account codes, or other details. However, a Change Order
cannot
be used to change the vendor—if a vendor change is necessary, a new requisition and Purchase Order must be issued.
