Only full time positions are eligible - While filling out this request form, please indicate whether this request is for a newly created position or if it is to fill an existing position. 

Important Note: Please submit this request no later than 5 days in advance of the employee's start date. This will allow OIT the appropriate amount of time to allocate / prepare the equipment.
